Chronic Condition Care

Chronic Condition Management in Leominster, MA

Living with a long-term condition is easier when the person reviewing it already knows your history. ChristCare Wellness Center provides continuing care for chronic conditions, with enough appointment time to do it properly.

Continuity

The same provider, visit after visit

Long-term conditions are rarely solved in a single appointment. They are managed, reviewed and adjusted over years. That works best when you are not re-explaining your history to someone new each time.

This is a relationship-based practice. You see the same provider, your record is read before you walk in, and changes to your treatment are made with the full picture in view.

What Ongoing Care Looks Like

Review, adjust, follow up

  • Scheduled reviews rather than appointments only when something goes wrong
  • Medication review, including how your prescriptions interact with each other
  • Laboratory monitoring at sensible intervals, with the results explained to you
  • Lifestyle support: nutrition, movement, sleep and stress, addressed as part of treatment
  • Referral and coordination with specialists when a condition needs them
  • Care in English, French, Haitian Creole, with Spanish-speaking patients welcome
Between Visits

Knowing when to call

Part of managing a long-term condition is knowing what is expected and what is not. You will leave an appointment clear on what to watch for, what to do if something changes, and when to call the office rather than wait for the next review.

For a medical emergency, always call 911.

Common Questions

Questions we are asked most

Which long-term conditions do you manage?

ChristCare Wellness Center provides primary care management for the long-term conditions commonly seen in family practice. Call (978) 775-2067 to confirm that your particular condition is one we can manage, and we will tell you plainly if specialist care would serve you better.

Can you work with the specialists I already see?

Yes. Where a condition is already under specialist care, we coordinate rather than duplicate, and keep your primary care record complete.

How often will I be seen?

That depends on the condition and how stable it is. The intention is scheduled, predictable review rather than appointments driven only by flare-ups.

Do you take new patients with existing conditions?

Yes. The practice is accepting new patients, including people already living with long-term conditions. Call (978) 775-2067 to arrange a first visit.
